Annual Permission Form and Record Update Form (Required)

SLPS requires a parent/guardian to submit an Annual Permission Form AND Record Update Form for each returning student.

If a parent/guardian fails to submit an Annual Permission Form for their student(s) before the 2023-24 school year starts, it will be assumed the parent/guardian is opting in and agreeing to all permissions in the affirmative.  Once the Annual Permission Form is completed, the district will adhere to the submitted wishes.

Both forms are now available in the SL PowerSchool Parent Portal (Link) and the PowerSchool Mobile App (iOS / Android).

  1. Log into SL PowerSchool with your Parent Account.
  2. With multiple students, choose your first student from the names at the top.
  3. On the Parent Portal, click the Forms option on the left navigation menu.  On the Mobile App, press the More button at the bottom and choose Forms.
  4. Click the Permissions and Annual Review form link, make selections and Submit the form
  5. Click the Record Update form link, make any necessary changes and Submit the form (Click the small triangle for an option to submit for all students)
  6. Repeat this process for any other returning students (return to step 2)

SchoolMessenger

Try out the SchoolMessenger app (go.schoolmessenger.com).

SLPS uses SchoolMessenger to notify parents/guardians of school and district updates.  SchoolMessenger is also used to communicate unresolved absences with an evening phone call.

If you are not receiving messages at the start of the school year, please verify your notification phone and email are accurate for your student(s).  The notification phone and email fields are available in SL PowerSchool in the Record Update Form.

Please check the spam/junk box in your email for any missing messages.  Choose those messages and mark them ‘not spam’ or ‘not junk’ so future messages are not hidden.

If you are still not receiving messages, please contact your students’ school office to verify your email address has not been unsubscribed.
